InGenesis is currently seeking a dedicated Community Health Worker II (CHW II) to join our team. This role is vital in connecting patients with essential health and social services across our clinics and community outreach events. As a trusted liaison, the CHW II will support care coordination, patient engagement, and resource navigation to improve health outcomes for our diverse patient population.

Key Responsibilities
- Serve as a bridge between patients and health/social services within the clinic and the broader community.
- Conduct outreach, engagement, and care coordination activities.
- Assist patients with enrollment in health plans (CEC certification required).
- Analyze screening results and collaborate with the Care Team.
- Provide crisis intervention, advocacy, and problem-solving support.
- Maintain accurate and confidential documentation in client files.
- Participate in team meetings, case conferences, and training sessions.
- Support grant deliverables through assessments and data collection.
- Educate patients on health topics such as nutrition, hygiene, and substance abuse.
- Work in varied settings including clinics, schools, mobile units, homeless encampments, and patient homes.
- Attend workshops, trainings, and meetings as needed.
- Travel to other BACH locations within a 50-mile radius.
- Perform other duties as assigned by supervisor.
Qualifications
Required:
- High school diploma or equivalent.
- 1-2 years of experience in community-based healthcare preferred.
- CHW Certification (with recertification/retraining as needed).
- CEC Certification.
- Proficient in computer and internet-based applications.
- Bilingual skills preferred.
Success Factors
- Strong adaptability and comfort with technology and internet-based tools.
- Ability to connect patients with services and enroll them in health plans.
- Knowledge of preventive care and treatment protocols for high-risk populations.
- Skilled in health navigation and care coordination.
Work Conditions
- Travel to various sites and patient homes required.
- Exposure to outdoor environments and varying temperatures.
- Prolonged periods of sitting or standing.
- Physical activity including lifting, bending, and kneeling may be required.
