InGenesis is seeking a Community Health Worker III to work with our client.In this role, you will conduct outreach and engagement, development of the care plan, and assist in the coordination of health and social services.

Essential Responsibilities:
- Advocate for patients and help them achieve their best health.
- Be highly dependable and independent - much of the work will take place in various settings -medical offices, specialists, hospitals, and patients' homes.
- Communicate well with patients, document clearly in the medical record, and interface with medical providers to advocate for the patient.
- Perform patient screening surveys.
- Serve as a liaison between the community and health and social services.
- Interact with clinic staff and the community.
- Collect data and provide documentation in client files.
- Participate in obtaining deliverables required for various grants.
- Assess patients' home living situation.
- Conduct SDOH assessments and provide resources as appropriate.
- Schedule staff for other external or online training and assist with providing hands-on training and development.
- Support patient engagement, linkage, coordination, monitoring, follow-up, and advocacy.
- Develop and periodically update an inventory of available resources for patients.
Minimum Qualifications:
- Highschool diploma or equivalent.
- As/AA in health or related field preferred.
- 2-4 years of experience in a community-based health care service capacity preferred.
- Bilingual preferred.
- CHW Certification
- CEC Certification, if applicable.
- Cultural competency working in diverse/low income and immigrant communities.
- Knowledge of population related to specialty care and other health-related services.
- Ability to work in a fast-paced environment as part of an interdisciplinary team.
Work Conditions
- Travel to other sites and/or locations required.
- Work outside in varying temperatures may be required.
- Prolonged periods of standing or sitting.
- Work flexible hours, which include some evenings and weekends.
